Drug errors can occur in a multitude of ways, just a few listed below.

Prescribing error by a GP or Hospital Doctor

With the best will in the world, prescribing errors and dispensing mistakes happen. Sometimes a doctor may write the wrong drug on a prescription (a prescribing mistake), or the right drug but at the wrong dosage.

Dispensing error by at a Chemist or Pharmacy

At other times, a chemist will dispense the wrong drug, or the wrong dosage, by misreading a prescription (a dispensing mistake). Often the harm to a patient is short lasting and mild, although unpleasant. However, sometimes such errors can have far more serious consequences.

Misprescribing

Various drugs can be prescribed for too long, in too high doses, or there can be a failure  by a GP or Hospital to properly monitor a patient taking a medication, to ensure they are not suffering from side effects that can lead to injury, at times irreversible. There are a few examples below:

Benzodiazepines

Our Director, Caroline Moore, is known for her work in the field of benzodiazepine litigation. Generally, such drugs should only be prescribed for acute episodes of severe anxiety, or very short-term use for sleep disorders, and in small doses for some epilepsy patients. However, unfortunately, there continues to be misprescribing falling outside the recommended guidelines, resulting in patients becoming long-term addicts, through no fault of their own.
